Swenson Healthcare Selects Puget Sound Community to Launch New Post-Acute Services Group

Physician leader brings broad expertise to patient care

TACOMA, Wash.--()--Swenson Healthcare PLLC is pleased to announce the launch of a new post-acute care clinical services group in the Puget Sound area. After a nationwide search for the most desirable professional and personal setting, founder Darren Swenson MD selected this community to apply his experience, training and leadership to bring a new dimension of quality care to post-acute clinical medicine.

“Having a daily physician presence in our center helps our patients adjust to the transition from the hospital setting to a lower level of care”

Currently selecting and training clinical staff, Swenson Healthcare is partnering with skilled nursing and long-term care facilities throughout the Puget Sound region. Swenson Healthcare’s approach is to assess the totality of a patient’s needs, and to create a care plan in coordination with clinical colleagues and family. A true independent practice, Swenson Healthcare is dedicated solely to post-acute care in the cities and towns of the Puget Sound area. The Swenson Healthcare motto, “Caring for Our Community,” emphasizes Dr. Swenson’s conviction that local and private clinical practice is the surest and safest way to provide the best quality patient care in a post-acute setting.

Dr. Swenson is triple Board-certified by the American Board of Internal Medicine, the American Board of Quality Assurance and Utilization Review Physicians, and as a Certified Physician Executive. His broad experience includes risk-based managed care programs in both acute and post-acute care facilities. With over twenty years of clinical experience as an internal medicine physician, Dr. Swenson remains passionately focused on the quality, safety and effectiveness of his patients’ care. Commenting on Swenson Healthcare’s objectives, Dr. Swenson says, “We aspire to elevate the patient’s experience and clinical outcomes in the facilities where we care for patients and provide medical director leadership.”

“Having a daily physician presence in our center helps our patients adjust to the transition from the hospital setting to a lower level of care,” states Stacy Mesaros, Senior Administrator for ManorCare Health Services in Tacoma. “Dr. Swenson and his team spend time at bedside and with our clinical team to ensure patients’, families’ and health plans’ goals are being met and the patient is able to transition to the community quickly and safely while reducing the likelihood of a return to the hospital.”
