Community Health Network Adds Employer-Directed Population Health Program in Partnership with Aegis Health Group

INDIANAPOLIS--()--Community Health Network , ranked among the nation’s most integrated healthcare systems, has partnered with Aegis Health Group to improve the health of local workers and community organizations by implementing employer-based wellness initiatives supported by OneCommunity, a population health management website. Through this program, the health system will work with area businesses to identify health risks among their employees and offer appropriate hospital-sponsored programs designed to promote better health. The interactive OneCommunity web solution, powered by Aegis, will also be used by Community Health Network (CHN) employees supporting the provider’s efforts to build a healthier workforce.

Employer Relations Specialists will oversee the program using proprietary tools including the OneCommunity website. It is designed to match at-risk consumers with appropriate early detection, prevention and education initiatives offered by the health system. It also engages them with their own password protected, online health dashboard featuring content tailored to their health status. Employers will gain an unprecedented perspective of their workforce health risks. CHN will then target health programs designed to positively impact employee health and reduce health claims. “We know that to make this program successful both internally and externally, we have to make it easy and engaging for users to get important information and guidance,” adds Zetzl, “and OneCommunity makes this possible.”
