The health care fraud occurred over a five year period, since the business first opened and throughout its entire existence. The client submitted claims with obtained Medicaid identification numbers for transportation services not provided in conjunction with false mileage claims.
The client and spouse gambled at casinos, lived lavish lifestyles, and owned three homes. The healthcare business operated out of another large 8 room home, in a quiet rural area, surrounded by lush vegetation and trees.
