WASHINGTON--(EON: Enhanced Online News)--The first three organizations have achieved Case Management (CM) Accreditation from the National Committee for Quality Assurance (NCQA): Alere, Monroe Plan for Medical Care and OptumHealth.

Designed for case management programs in provider, payer or community-based organizations, NCQA’s CM Accreditation ensures that patients with complex conditions receive effective, coordinated patient-centered services that lead to good outcomes. By achieving NCQA Case Management Accreditation, organizations make a clear statement that they are dedicated to providing personalized, effective care—not just checking a box for another distinction.
The NCQA CM program is unique for assessing quality during a patient’s transition between care settings. Since patients’ health needs often fall through the cracks when they move from one setting to another, effective case management can lead to continuity of services and sharing critical information as patients travel through the system.
To qualify for the NCQA CM Accreditation program, organizations must manage care for patients with complex health care and social needs. The early adopter designation means Alere, Monroe Plan for Medical Care and OptumHealth completed their NCQA CM Accreditation survey by July 31, 2013.
